The Extended Care Rider Features
When you add this rider to the Hospital Indemnity Insurance Plan, you’ll receive additional benefits that may help
pay for up to 200 days of home healthcare, skilled nursing facility and nursing home after you’ve been
hospitalized.6

Coverage details
- Available to all eligible ADA members and spouses under the age of 65, and dependents under age 21 (or 27 if a full-time student)
- The Extended Care Rider requires medical underwriting
1Premiums are effective as of 4/1/2020 and include a 45% Premium Credit discount that shares
favorable financial results with Plan participants; discount is not guaranteed but reevaluated periodically.
The Extended Care Rider provides a daily benefit for up to 200 days.
2At age 70, Hospital Indemnity Insurance plan coverage maximum is $200 per day for all participants.
3Beginning at age 65, benefit amounts can no longer be increased. Hospital Indemnity Insurance Plan Coverage
is renewable up to age 90.
4Hospital Indemnity Insurance Plan claims are not payable for any confinement or treatment resulting from
a condition for which the insured received any medical treatment, care, advice, or medication within 12 consecutive
months before the effective date of this insurance and applicable to any insurance increases.
5Members who are under age 65 with unmarried children under age 21 (27 if they are full-time students)
can apply for dependent children coverage. The monthly cost for each dependent child is $1.74 per $100 of benefit.
6Benefits for confinement in a Skilled Nursing Facility or Home Healthcare will be payable only if
such post-hospital confinement begins within seven days after at least three consecutive days of confinement in
a hospital.
